PSR 5 Way Switch Wiring

I have a DW CE 24 with the stock 5 way blade and was hoping someone could help me out with this wiring/configuration.

Position 1: Off
Position 2: Bridge Humbucker
Position 3: Bridge Humbucker & Neck Single Coil
Position 4: Both Single Coil
Position 5: Neck Humbucker

I having some difficulty figuring out the super switch.
