Update: the guitar is on its way to the PTC.

I was hoping you'd do a neck adjustment as it sure seems like that would cure your issues.

I would have made a truss rod adjustment. Since your action was significantly higher at 12th fret a truss rod adjustment SHOULD have flattened that neck out a bit forcing you to raise strings at saddles and or trem to get desired action on the entire neck.

You're right, except as I mentioned in an earlier post, the neck relief is at 0.006". It can't get much more flat.
