Need help figuring out production year

Hey, I have a PRS custom 22 that I bought sometime in the mid 2000's.

I have the hang tag it says serial # 91360, the 9 meaning it's either 1989 or 1999 and 1360 shows that the neck was made in 1986?
I'm a bit confused since the back of the headstock it writes 4 91360, would that mean it's made in 2004?
 
Those would be Phase 2, making it a 2004.
Agreed, here’s my 2004 HB Spruce. My s/n is 4 86xxx.

The serial number actual can generally tell you what year it was made. At the link someone posted above, https://support.prsguitars.com/hc/en-us/articles/4408314427547-Year-Identification look at the "Set-neck Serial Numbers table" (which is just below the Serial Number Year-Code Prefix table you referred to).

As you and everyone else confirmed, the "4" means it's either a 1994 or 2004.

In the Set-neck Serial Numbers Table, "91360" falls in the range of 2004 (82255- 92555).
